Benefit orientation Informal Management Referrals (Recommendation) HR/Supervisor recommends that the employee contact EAP, and provides the EAP benefits and accessing instructions to the employee ( ) Employee decides whether or not to contact EAP. EE decides NOT to contact EAPEE decides to contact EAP EAP Counselor deals directly with Employee. HR/Supervisor continues to monitor job performance per their policy. If problem continues, HR/Supervisor will move to a more formal referral process. Supervisor has no way of knowing if the EE contacted the EAP.

Release of Information is sent to the Supervisor making the call and the ORMC must receive the ee signed document from HR/Supervisor prior to implementing the EAP referral. Employee meets with the Supervisor/HR and given the directive to call the ORMC at and speak to xxxxxxx. The employee must sign the release and is given a deadline to call the ORMC. The Supv/HR is to fax or scan and the signed release to the Management Consultant handling the case.
