UTMB History and Facts UTMB Academic & Research History & Facts  The state’s first Academic Health Center Medical School established in 1891 celebrating 125 th anniversary in 2016  We are 9 th in the US in the number of MD graduates with an active license to practice medicine.  There are 29,000 living alumni; 21,000 in Texas  UTMB is Home to the only national lab in Texas and one of the two national biocontainment labs in the U.S. dedicated to developing treatments, vaccines and diagnostics to combat infectious threats to health, such as Ebola.  UTMB’s vaccine development center is one of the world’s largest university- based and has been named a World Health Organization (WHO) Collaborating Center for Vaccine Research, Evaluation and Training on Emerging Infectious Diseases. 2

RFP Section 1 – HUB Plan The Statewide HUB Program is under the direction of the Comptroller of Public Accounts (CPA) which administers, certifies, audits and reports HUB participation within the state’s procurement processes. In accordance with Texas Government Code 2161, each state agency, including UTMB, is required to make a good faith effort to assist HUBs in competing for, and receiving, contracts awarded by the agency. UTMB has reviewed this RFP in accordance with the Texas Administrative Code Section (Title 34, Part 1, Chapter 20, Subchapter B, Rule 20.14) and has determined that subcontracting opportunities are not probable under this solicitation. If your company does intend to subcontract any portion of the work, your proposal must include a “HUB Subcontracting Plan” (HSP). RFP Section 3 – General Terms & Conditions The General Terms and Conditions contained within the RFP Document shall be made a part of and govern any Purchase Order, unilateral or bilateral Agreement, if any, resulting from this Request for Proposal. In addition, UTMB has attached a proposed an Inpatient Care Agreement for review. 10 Purchasing

RFP Section 4 – Scope of Work / Service Requirements General Requirements of Respondent Provide nurse/social work/chaplain services PRN by hospital request 24 hours a day. Provide on-call services 24 hours a day. Once patient makes General Inpatient Hospice admission election, hospice team member must arrive at hospital within two hours to complete psycho/social nursing assessment. Upon admission to General Inpatient Palliative Care Service, hospice agency nurse will make initial visit within two hours to conduct intake evaluation. Hospice agency designee shall conduct daily onsite visits to assess patient conditions and discuss plan of care with hospital physician attending. Hospice agency will provide, if death occurs in General Inpatient Palliative Care, decedent support for family within 30 minutes from notification of death. Payment for services rendered is at 90% daily Hospice GIP per Diem. Hospice agency must currently be Community Health Accreditation Partner (CHAPS) certified and maintain certification for duration of medical services agreement. Hospice agency must be in good standing with state regulators. Hospital will provide: Room and board (dependent upon bed availability) Nursing staff Pharmacy services UTMB Palliative care physician medical attending services 11 Purchasing
