DETAILS OF THE INCIDENT Date of the Incident: 17/09/2008 Place: Medium B Time of Incident: 14:35 Type of Incident: Assault / Death Number of victims:7 Offenders / 1 Official Number of perpetrators involved in the incident: 10 Offenders

DESCRIPTION OF INCIDENT On 2008/09/17 at about 14:35. Ten 26 gangsters attacked other offenders during the exercise time. A fight erupted. Unit members they got held up in the middle of the fight and could not handle the situation. Help was summoned. Members disarmed the offenders. Offenders that were fighting back calls themselves the Forums / Peace Committees.

INCIDENT REPORTED Date reported to SAPS:17/09/2008 Case number: CR108/09/08 Regional Commissioner:17/09/2008 National Commissioner: 17/09/2008 Minister:17/09/2008 SMS messages was utilized for reporting as well as a written report.

IMMEDIATE PRECAUTIONARY MEASURES All offenders in the Centre were locked up. Movement of offenders were restricted. Meeting with all staff conducted on 18 September Addressed by the Area Commissioner. Head of Correctional Centre implemented various security measures from the Contingency Plan. Searching of the Correctional Centre took place. Mass searching took place on 25 June 2008 and 26 June 2008 by the officials of the EST. A mass searching of the Centre is planned.

ANTICIPATED DISCIPLINARY MEASURES The matter was handed over to the SAPS at Westville. Four officials were appointed as Investigators, namely: Mr. J Stuart (DD) Mr. B Lambert (ASD) Dr. G Govender (SCO) Mr. S Mthiyane (COG I)
