Recommendation #1 “Warehouse workers do not perform any duties outside the scope of their normal assignment. If the employer had policies and procedures that would have prevented the victim from servicing the air conditioner, this incident would have been prevented.”

Arc flash In Los Alamos report - unclear if there was a lamp or other visual indicator or voltage in cabinets to warn users.

Recommendation The Team identified the overall root cause as less-than-adequate management of control implementation, which the Team considered a combination of two specific root causes: ● Failure to require and implement zero-voltage checks; and ● Lack of established physical barriers.
