Transforming Care An approach to measure, track and help achieve the core objectives of Transforming Care It will help you see the effect of the changes you’re making It will enable you to use facts – not gut feel - to make decisions and drive improvement It will help engage management and encourage their support Knowing How We’re Doing (KHWD)

Transforming Care “In God we trust - all others must bring data” If something’s important, you must measure your performance on it. How do you think wards who have successfully prevented pressure ulcers started? Data plays several vital roles in helping us improve performance: –Lets us know how good or bad things are –Helps us look for patterns e.g. do we get more falls on the weekend? –Helps us prove our approach has worked e.g days since a pressure ulcer on Anglesey ward

Transforming Care KHWD Board Displays your measures and your ward vision Meetings round the board each week Keep track of performance and look for opportunities to improve Assign ownership of actions and review progress Keep it simple – 6 measures is enough until you’re very used to doing this

Transforming Care KHWD Exercise In your groups, refer to your ward vision and answer these questions: What 3 things could you measure that would help you to see whether you are achieving your vision? How could you collect this information? How would you keep track of it? How would you use it to drive improvement?
