Removal of a Safety Barrier During preparation of the He spill test it has been agreed by all participants to leave the access points in General Mode and to prevent unauthorised entry by barriers. It was also agreed that FRS would use PM 56 to bring their equipment trailers underground 38th LSC, Thomas Otto, Safety Officer in charge of LS :00

Removal of a Safety Barrier On 4.2. at 19:55, two FRS staff members temporarily remove the barrier to use the PAD more easily. At 20:45, they forget to put the barrier back in place, in spite of the presence of anr FRS supervisor. At 21:00, an HSE staff member does not see a barrier and uses PM 56 to enter the potentially hazardous underground area. 38th LSC, Thomas Otto, Safety Officer in charge of LS :00 Given the importance of reliable safety barriers, a detailed investigation and report have been tasked to John Pedersen, EN DSO.
