The state of cancer survivor care planning Comment 1) The importance of survivorship care planning is well-recognized NAP for cancer survivorship 2) The elements of care plans are well described and accepted Lost in Transition 3) Survivorship Care Plans are available and are being adopted ASCO, CDC BRFSS care survivor module 4) Implementation and Evaluation of survivorship care plans raises many questions Implementing survivorship care planning

Project Review CriteriaMeets CriteriaComment 1) Clearly advances dissemination science √1)Extends the knowledge base of translation by addressing questions such as: to what extent, to whom, how, and with what outcomes are survivorship care plans being implemented? 2)By studying places where survival care plans are being implemented successfully, we can help inform strategies for promoting wider scale adoption 3) Address a vital gap in evidence that will become more critical if survivorship care plans are mandated as part of quality cancer care
