In Preparation for 7 th Grade: State immunization requirements:  Tetanus/Tdap booster  Meningitis vaccine  2 nd Varicella vaccine or history of Chicken Pox disease  If you are unsure about your child’s immunization status, contact your doctor’s office. Be sure to tell them your child is going into the 7 th grade to ensure they are compliant for next year’s requirements.  The immunization records can be submitted to the clinic via fax, , or in person beginning now!  Please note: Do not send immunization records to the coaches with a physical OR send athletic physical forms to the nurses! Athletics: District Wide Physicals will be offered Saturday, May 14th 8:00-9:30 AM, at Highlander Stadium. Cost is $20.00 with checks payable to HP Sports Medicine.
