Paraburdoo Mining Operations Light Vehicle Haul Truck Collision Initial Team Briefing

Sequence of Events At approximately 0500 hrs on the driver of an MT4000 haul truck requested to be changed out for a break The driver of the haul truck parked the truck clear of the loading area. A relief operator in a light vehicle approached the truck and parked the light vehicle in front of the truck The relief operator boarded the truck The haul truck operator dismounted the truck and accessed the light vehicle

Sequence of Events Before the light vehicle was clear of the area the haul truck driver proceeded forward running over the light vehicle The haul truck stopped with the light vehicle upside down between the truck wheels The mayday procedure was activated and the emergency response team attended the incident The driver was removed from the vehicle after approximately two hours of extrication and transported to the Paraburdoo hospital. The driver was conscious throughout the incident

Actions The driver of the light vehicle has been air lifted to Royal Perth hospital for treatment of minor fractures and bruising. He is expected to make a full recovery. All teams across Pilbara Iron mining operations have received initial briefs and will receive this presentation at the first opportunity Prime support is being offered to all involved in the immediate incident This incident re-enforces the requirement to follow safety procedures at all times A thorough incident investigation is underway and finding will be communicated
