MSHA’s Fatality Prevention Program

MSHA’s commitment to eliminate hazards with serious consequences

Targets prevention of conditions that cause or contribute to fatal accidents

2000 – miners died – 300 Coal fatalities – 289 MNM fatalities

Fatalities 47% of the fatalities were related to these 11 Coal standards. 47% of the fatalities were related to these 11 Coal standards. Five Coal standards for surface mines. Five Coal standards for surface mines. Six Coal standards for underground mines. Six Coal standards for underground mines.

Surface Mine Standards (c) (c) Repairs or maintenance shall not be performed on machinery until the power is off and the machinery is blocked against motion, except where machinery motion is necessary to make adjustments.

COAL MINE FATALITY - On Wednesday, March 12, 2008, a 47-year old mechanic/contractor with 21 years of experience was fatally injured while attempting to remove a front guard assembly (belly pan) from a bulldozer. The victim was positioned beneath the bulldozer, removing mounting bolts from the bulldozer guard. The guard was supported by a chain attached to a remote-operated truck-mounted crane. As the bolts were removed, the chain support slipped and the guard fell.

Surface Mine Standards (g) (g) Equipment operators shall be certain, by signal or other means, that all persons are clear before starting or moving equipment.

COAL MINE FATALITY - On Monday, July 2, 2007, a 59 year old bulldozer operator with over 35 years experience was fatally injured when he was struck by a pickup truck. The victim and two other operators were near their equipment when a foreman instructed them to move in preparation for a blast. The victim was apparently walking back to his bulldozer when the foreman, who was using the truck's side mirrors to drive, backed over him.

Surface Mine Standards (n) (n) Mobile equipment shall not be left unattended unless the brakes are set. The wheels shall be turned into a bank or berm, or shall be blocked, when such equipment is parked on a grade.

COAL MINE FATALITY - On Thursday, July 20, 2006, a 39-year old road grader operator with 20 years of mining experience was fatally injured on the haul road of a surface coal mine. While the victim was outside the cab of the parked road grader, it began to roll backwards down grade. The grader came to rest along the hillside ditch line with the victim positioned under the rear ripper-bar.

COAL MINE FATALITY -On Monday, July 30, 2007, a 27- year old roofing contractor was fatally injured when he fell approximately 28 feet through the roof of a maintenance shop. The victim apparently either stepped on a fiberglass skylight, causing it to break, or fell through an opening adjacent to the skylight while removing old roof material.

Surface Mine Standards (i) (i) Seatbelts in a vehicle where there is a danger of overturning and where roll protection is provided.

COAL MINE FATALITY - On Friday, February 6, 2009, a 70 year old contract truck driver with eight weeks experience on this job was fatally injured while driving a truck on a coal haulage road. The truck struck the left embankment at an angle, traveled approximately 97 feet down grade, and turned over on the road. The victim was fatally injured when he was trapped under the cab of the truck.
