WHAT CAN YOU DO WITH GOOGLE FORMS? According to Google’s (2014) website, you can create a wide variety of forms to: Plan events; Send surveys; Give students a quiz, and; Collect data.

RESPONSES 1.Responses are collected and summarized in a spreadsheet. 2.You can also view the responses in real-time. 1 2

LOGIN TO GOOGLE DRIVE Create an account with Google drive.google.com OR Log in to your Google account drive.google.com

STEPS TO CREATE A FORM Click on Create Click on Form Then complete the following steps: Type a Title Select a Theme Click on OK

ADDING A QUESTION 1.Type the Question Title 2.Type Help Text (optional) 3.Select the Question Type Text, Paragraph, M/C, List, Scale, Grid, Time. 4.Type the correct answer (if applicable). 5.Click on Done

PRACTICE TIME! CREATING ENTRY FIELDS 1. Type the Field Name. Example: First Name 2. Select Text as the Question Type Edit Duplicate Delete Click on the Duplicate button to recreate the same type of question repeatedly

PRACTICE TIME MULTIPLE CHOICE QUESTION 1. Type a question 2. Type help text 3. Select question type 4. Type the answers 5. Click on Done

ADD ITEMS Click on Add Item & Repeat to add more questions

CONFIRM YOUR RESPONSES Click on Send Form & Repeat Copy the Link to Share Copy the Link to Share or Send via Click on Done

VIEW RESPONSES View responses View how many responses you get in real time A new tab will open up with the responses collected on a spreadsheet

VIEW LIVE FORM Click on View live form for a preview Live Form Preview

RETURN TO GOOGLE DRIVE Once you have completed your form, click on the Open Google Drive link.
