Vision Statement Every young adult will exit transition programming with the ability and confidence to make choices and take action to make those choices happen.

Guiding Principles Every young person has a voice that is valued and respected. Key stakeholders play a critical role. Services are driven by transition domains. Services are efficient and cost effective and based on data. All staff engage and support all young adults. Young people are provided services based on identification of need not eligibility criteria or base list.

An ILP is a student focused meeting with stakeholders to develop a plan. The plan is a work in progress to help the young adult determine their vision for the future and successfully transition from programming to independent life skills.

STAKEHOLDER TEAM Identified primarily by the young adult. Anyone with a vested interest in the young adults future. Ex. Case manager, parent, other relatives, group home, community agencies, minister, peers and etc… Stakeholders give input, but do not determine priorities of the ILP.

Stakeholder team and Staff are facilitators of the ILP and drive it but they don’t determine it. Insert ILP meeting video clip ILPs become the primary focus of and drive the activities of the day ICE data is collected on skill attainment and/or level of prompts needed.

Intentional Community Experience - ICE are developed to use, practice or demonstrate skill will be used to collect data on the acquisition of skills (they show us they have it) with or without jigs.

INTENTIONALITY/DATA COLLECTION All staff is engaged in intentional learning experiences. Intentionality comes from specific data collection vs. anecdotal Ex. Fine tuning current collection tool
