TOM TORLAKSON State Superintendent of Public Instruction CHKS Assesses health risks, specifically relating to alcohol, tobacco, and other drug (ATOD) use; school violence; physical health; resilience and youth development; and school climate. Available as a web-based online version or in paper form. 2

TOM TORLAKSON State Superintendent of Public Instruction CHKS Allows schools and districts to monitor whether they are providing the critical developmental supports and opportunities that promote healthy growth and learning. 3

TOM TORLAKSON State Superintendent of Public Instruction CHKS First administration of CHKS in Delhi in almost 10 years. CHKS data will be used to inform the district’s LCAP priorities. 4

TOM TORLAKSON State Superintendent of Public Instruction Parental Consent Parent consent forms will be sent home on February 8, 2016 and are due on or before February 16, th grade consent form is active. –Teachers submit returned forms daily to the site CHKS Coordinator.

TOM TORLAKSON State Superintendent of Public Instruction Parental Consent 7, 9 11 grade consent forms are passive. –Watch for “non-participation” forms. Some parents may not want their child to take the CHKS. These forms are submitted to the site CHKS Coordinator.

Parents: 2/1/ /29/2016 –Online and paper versions will be available. Staff and parent online surveys are prominently posted to the district’s website under the Special Programs, Assessments, Student Services webpage. 10
