Plan 1 Your employer has selected a specific benefit amount to complement your medical plan:  $500–$5,000 for each covered hospital admission [In addition, your employer has chosen to include:  [$100–$400 for each day of your covered hospital stay, up to 15 days]  [$200–$800 for each day you spend in intensive care, up to 15 days]  [$150 for emergency room treatment for an accident (once per year)]  [$100 for ambulance or $500 for air ambulance transportation (once per year)] Covered health events and lump-sum benefits

Plan 3 Your employer has selected a specific benefit amount to complement your medical plan:  $500–$5,000 for each covered hospital admission  $250–$1,000 for physician-ordered diagnostic procedures (once per year)  $500–$2,000 for outpatient surgery, depending on the procedure, to a calendar-year maximum  $250–$1,000 for covered inpatient surgery (once per year)  A travel benefit of $0.40 per mile, for up to three round trips (1,200 miles per round trip), if you meet the following guidelines:  You must have a physician’s referral  You must have a hospital stay or outpatient surgery  The trip must be at least 50 miles one way  A lodging benefit of $150 per night to lodge a companion during your hospital stay, up to 30 nights Covered health events and lump-sum benefits

12 Wellness Benefit Based on the plan selected by your employer, this benefit can pay $50, $75, $100 per calendar year per insured individual if a covered health screening test is performed. Screening tests include, but are not limited to:  Colonoscopy  Mammography  Colonoscopy  Skin cancer biopsy  PSA (blood test for prostate cancer)  Chest X-rays  Stress test on a bicycle or treadmill

13 In addition to employee coverage, you can choose the following coverage options:  Employee & spouse  Employee, spouse & child  Employee & child Eligibility Spouse – Ages 17 to 64 with purchase of employee policy Children – Available with purchase of employee policy for eligible children and stepchildren, newborn to age 26,* regardless of marital or student status. Family coverage options * In IL, dependent child(ren) covered to age 30 if actively enrolled in the military.

14 Pre-existing condition limitation: Unum will not pay benefits for a claim that is caused by, contributed to by or occurs as a result of a pre-existing condition† Policy provisions †A pre-existing condition is a sickness or injury or symptoms of a sickness or injury, whether diagnosed or not, for which you received medical treatment, consultation, care or services, including diagnostic measures, took prescribed drugs or medicine or had been prescribed drugs or medicine to be taken during the 12 months just prior to your coverage effective date. ††Varies by state. 15  Hospital admissions can cause serious financial setbacks, due to out-of-pocket costs that are not covered by a typical medical plan.  Hospital indemnity benefits are paid in a lump sum, not as cost reimbursement.
