Change of Name in the Same Precinct NOT A PROVISIONAL VOTER! If the Voter has changed his or her name and provides proof of the legal name change (e.g., a marriage license or a court order that includes the Voter’s current and prior names), the Voter may complete and sign a Notice of Change of Name (Form 10-L) and cast a regular ballot, as long as they are registered to vote in that precinct.

NOT A PROVISIONAL VOTER! Change of Address in the Same Precinct If the Voter has changed their address to a new address in the SAME precinct, have them fill out a change of address card and have them sign it. They may then vote a regular ballot.

Provisional Voters Who Votes a Provisional Ballot?

Voter’s from Another Ohio County If a Voter comes from Another Ohio County TO Mahoning County and wants to vote, they will NOT be on ANY list in your Electronic Poll Book. They may, however, vote. Call the Mahoning County Board of Elections ( ) and give a Staff Member the Voter’s NEW address. They will give you the information to direct the Voter to the correct Polling Location or Precinct. The Voter will NOT get a Voter Print Ticket. Follow all procedures for the Provisional Voter at the Precinct Table.
