Directions Administer 1 on 1 Each assessment should be completed at the end of the month stated Provide manipulatives as needed Use checklist to monitor student progress Feel free to adapt questions

Question 1: Number Sense Start at 1 and count as high as you can –Stop the student if they reach 100

Question 2: Number Sense Count by 10’s to 100

Questions 3-5: Number Sense/Geometry Provide students with counters or any available manipulative Tell the student to show the number 4 using counters –If students show incorrect number, correct him or her before continuing to the next question Tell student to write the number 4 Have student draw an equal number of squares Repeat with the numbers 8 and 16

Question 6-8: Number Sense Show the number 5 Ask students to name the number that comes before and after the number 5 Repeat for the number 11 and 17

Question 9-10: Number Sense Using flashcards, have students order the numbers 5, 6, and 7 from least to greatest Repeat using the numbers 13, 14, and 15
