Date distributed. Method (annual report, parent handbook, mailed, sent home with child, etc.) Documentation of who the notice was sent to (attached list, all parents for building as of 9/15/06 list, etc)

b Title I Targeted Require- ments for Student Eligibility Based on Needs Assessment data, determine subject and grade levels for Title I. Develop with all teachers the 2 objective, academically related criteria used to identify students who are failing or at risk of failing reading and/or math (eg, test score and standardized teacher referral form). Determine a system to add the 2 criteria together. Rank students by the combined total criteria score from highest to lowest risk. Serve students in the order of list.

List of ranked eligible students showing values of criteria used and explanations of how calculated.

a Parent Involvement Policy District jointly develops and distributes written parent involvement policy (Doc Library, Parent, Consolidated App, District/LEA Parent Involvement Policy Template). Each Title I building does the same.

(All appropriate parapros must already be qualified.)
