OPEN ENROLLMENT 2013 PLAN DESIGN CHANGES (MEDICAL) –Elimination of the OAP PPO Plan –Employees currently enrolled in the OAP PPO plan must select a new plan –Employees who do not choose a new plan will automatically be enrolled into the New STANDARD HRA PLAN –Increase in Deductibles and Out of Pocket Maximums –Decrease in coinsurance levels –Introduction of new affordable health care plan –HMO plan out-of-pocket will now include copays, in addition to, deductible and co-insurance for medical only. DEPENDENT ELIGIBILTY AUDIT 11

−Purpose to ensure all enrolled dependents are eligible to participate in Shelby County’s Medical Plans. This process this mandatory for all employees enrolled for health care coverage. −The process is to audit every enrolled dependent as of December 1, −Eligibility audit will begin after open enrollment and completed in the first quarter of −If an employee knowingly has enrolled an ineligible dependent, the dependent MUST be removed during the 2013 Open Enrollment Period. −This is your only chance to remove your ineligible dependents without penalty. −Audit will be conducted independently by Shelby County’s Consultant, Buck Consultants. OPEN ENROLLMENT DEPENDENT ELIGIBILTY AUDIT IN 2014

–Buck Consultants will mail packets to the employees home address –If your address has changed, please Employee Benefits at or submit in writing directly to the benefits department –Employees will have access to Buck’s Customer Service Center and toll-free number –Employees may respond to Buck Consultants by Web, paper or fax –Ineligible dependents will be removed from coverage January 31, OPEN ENROLLMENT 2013 DEPENDENT ELIGIBILTY AUDIT ADDITIONAL INFORMATION

TOTAL HEALTH WELLNESS PROGRAM –To be enrolled in Shelby County’s Medical all employees will be required to take the Cigna Health Assessment which will include biometric screening information –The Cigna Health Assessment must be completed by September 30, 2014 –On-site screenings will not be offered. Employees will need to visit their physician to receive biometric screening *Reminder: Employees who successfully completed the Cigna Health Assessment in 2013, will receive a premium incentive beginning July 1, OPEN ENROLLMENT 2013
