FREE FLU VACCINATIONS AT SCHOOL November 5 th District Wide As part of an emergency preparedness exercise Goodhue County Health & Human Services will be offering free flu vaccinations to all students in Kindergarten – 12 th grade at Pine Island Public Schools on November 5 th. If you would like your child to receive a flu shot (IIV) or flu nasal spray (FluMist-LAIV) at school, please read these instructions carefully and review the information about flu vaccine at Then complete the online form at the link listed below. You will have the option to receive a confirmation message. The online form is submitted via a secure site and only Public Health staff will have access to it. The online form can be completed in the following browsers:  Internet Explorer versions 8 through 11  Chrome  Firefox  Safari You may need to turn off Compatibility Mode if the form does not display. If your child qualified to get the mist and it is not available, please check the box on the form if you would like your child to get the injection instead.
