Question Do the relatives you work with or others in your life have…. Hearing impairments, memory loss, limited mobility, diabetes, arthritis, osteoarthritis, cardiac conditions, high blood pressure, respiratory conditions, spinal cord injury, fibromyalgia, depression, stroke, asthma, multiple sclerosis, visual impairments, drug or alcohol dependence, Parkinson’s disease…

Question How do they feel living with a chronic condition? Scared? Frustrated? Fatigued? Angry? Depressed? Anxious? Sad? Confused? Uncertain? Isolated? Fearful?

Question What Can Be Done?

Well suited for people with different chronic conditions- focus on symptoms common to most chronic conditions Encourage clients/caregivers attend a CDSME workshop

Stanford University School of Medicine Patient Education Research Center Based on 1991 research study by Dr. Kate Lorig Goal: Increased autonomy so patients are active partners in managing their own health

Chronic Disease Self Management Program (CDSMP) 20+ years of proven impact -demonstrated results in variety of settings, populations & chronic conditions The “Gold Standard” of evidence based programming Offered locally and worldwide (24 countries) 7

Chronic Disease Self Management Assumptions People with different, ongoing health conditions  Have similar self management problems, tasks and concerns  Deal not only with their specific condition but its impact on their lives and emotions  Individuals can learn to take day-to-day responsibility for their conditions 8

Who Participates? Adults ages 18 and older who have one or more chronic conditions Family members, friends or caregivers of someone with a chronic condition Do not need an official diagnosis or have a doctor’s referral to attend No commercial products sold to participants No guest speakers No special equipment or materials 9

Proven Programs 2 ½ hours once a week for 6 weeks Peer led small groups (10-15 people) Trained co-leaders use highly structured and standardized curriculum Opportunities for discussion and problem solving Participants regain control of their lives Improved communication with health providers- partners in care 10

More confident, knowledgeable people who experience improved health, fewer days spent in the hospital and ER visits Participants self report 3 months-3 years after workshop: Improved social life/activities Improved energy/less fatigue Results

Diabetes Self-Management Program Originally developed in Spanish, Manejo de la Personal Randomized, controlled study in 2008 to assess workshop effectiveness for English speakers.

Diabetes Self Management Program People with type 2 diabetes attend the 6 week workshop Augments diabetes education-does NOT replace it Address techniques to deal with symptoms of diabetes, fatigue, pain, hyper/hypoglycemia Working with health care providers to manage diabetes. Other topics covered as in other CDSME workshops

Fees may be subsidized; Medicaid clients on COPES waiver $50/person/session paid to host organization-need DSHS case manager referral Convenience- workshops offered in community Credibility-designed by researchers at Stanford University; evidence based from study design to outcomes; all leaders take 32 hour training; ability to implement program to a wider population in various settings
