Mental Health Case management, Middle East International medical corps (IMC)

Psychiatrist Case Manager (social worker/trained nurse) Psychologist Trained PHC provider & Community Health Workers Case Management Team Structure The primary goal of case management is to optimize client functioning by providing quality services in the most efficient and effective manner to individuals with multiple needs. Case Management teams are usually health clinic based, with integrated outreach support & follow services

Psychiatrist Case Manager (social worker/trained nurse) Psychologist Trained PHC provider & Community Health Workers Community Based PS Other sectors (e.g. health) PHC level (walk in/ outreach) Upward & Downward referral Case Management starts with the Case Manager Not All Cases Require specialized Support Exceptional direct referral: Emergency cases 2 way referral & coordination
