ETP: Creating A Google Drive Reading Log Form Create A Reading Log Form On Google Drive Tsu-Hei Lin

Log into Google Drive:

Arrive at the intro screen of Google Drive.

Click on Create and select Form.

Type in a title for the reading log form and choose theme.

Enter questions to include on the reading log form.

There are different question types.

Check the box if the question must be answered.

Add more questions as needed. (refer to document: List of Reading Log Questions)

Decide choices for the confirmation page.

Click on “view live form” to see student view of form.
