Examples of good practice elsewhere

Examples of models elsewhere Monnow Vale Health and Care Centre Social care day services for older people, memory clinics, nurse led day hospital for older people, rehabilitation beds, integrated social care and community teams based at the centre Tewkesbury Community Hospital Day surgery, MIU, outpatients and 20 beds in single rooms, modern facilities and effective patient flow. New build for GP practices across the road from the community hospital.

Examples of models elsewhere Timberdine Nursing and Rehabilitation Unit Soon to be 36 bedded unit to include admission prevention beds, step down/rehabiltation beds and dedicated stroke rehabilitation beds with tracking for hoists. Many communal areas, single rooms, outdoor spaces and kitchen where food is freshly prepared. Jointly funded with the council.

Examples of models elsewhere Oxleas NHS Foundation Trust and Royal Borough of Greenwich Adult Community Services have developed Joint Emergency Teams. These comprise nurses, physiotherapists, occupational therapists and social workers. They operate 7 days a week and take referrals from GPs, A&E and paramedics. They provide an immediate response to prevent admission.

Examples of models elsewhere South Somerset are developing hubs for people with multiple long term conditions. These hubs will bring together primary care and secondary care (hospital based expertise) in one place. People will have a single care plan and co-ordinated care at the hub. In between visits people will be supported to remote monitor their condition with the use of technology. They will be able to view their care plan through a web platform.

Examples of models elsewhere Vitality Partnership in Birmingham and Sandwell is a GP partnership operating from 15 practice sites. They are developing a number of larger centres that will offer social care, mental health, community and secondary care support through community outpatient and diagnostic services.

Examples of models elsewhere North, Eastern and Western Devon are developing community health and well-being hubs. The aim is to provide an open access resource to the community. The focus is on staying well and preventing ill health. Examples of potential services the hubs will offer include: social activities, access to information, day care, memory cafes, GP referral schemes, exercise classes, nurse and therapy clinics, mental health support
