2 SCOPE This applies to all contractors acquiring to conduct meter reading, disconnecting and reconnection work throughout Eskom Distribution Eastern Cape Province. This covers the requirements for eliminating and mitigating incidents and injuries in all meter reading, disconnecting and reconnection contract

METER READING, DISCONNECTING AND DISCONNECTING RISKS The following high risk activities have been identified on the meter reading, disconnecting and reconnecting  Accessing meter cabinets (could results in bee stings, snake bites or contact with electricity).  Switching of MCB  To perform disconnections or reconnections of supply by means of the MCB  Driving of vehicle  Accessing customers premises could result with contact with dogs  Aggressive customer 3

LEGAL REQUIREMENTS Contractors shall comply, as a minimum, with all relevant legislation, South African National Standards, or international standards and with the client’s policies and procedures, which include, but are not limited to the following: The Constitution of the Republic of South Africa (particularly section 24 of the Bill of Rights) Occupational Health and Safety Act 1993 (Act 85 of 1993) and its regulations COID Act Applicable South African National Standards (SANS) Applicable international standards Operating Regulations for High-voltage Systems Plant Safety Regulations (Low-voltage Regulations) Construction SHE Management Procedures Incident Management Procedures Eskom SHE Policy Eskom Life Saving Rules It is the duty of the contractor(s) and their subcontractor(s) to ensure that they are familiar with all the necessary SHE legislation required for implementation on this project. 4

ESKOM LIFE SAVING RULES Eskom has implemented five rules that apply to all Eskom employees and contractors. These are as follows: Rule 1 – Open, isolate, test, earth, bond, and/or insulate before touch Rule 2 – Hook up at height Rule 3 – Buckle up Rule 4 – Be sober Rule 5 – Ensure that you have a permit to work These Life Saving Rules are non-negotiable health and safety rules that must not be broken under any circumstances. Eskom takes a ZERO TOLERANCE stance to violation of these rules. The client/agent is entitled to stop work and issue non-conformance reports whenever health, safety, or environmental violations are observed for both contractors and/or their subcontractors. Any costs incurred as a result of such work stoppage and standing time shall be for the contractor’s account. Any non-conformances/findings/observations found during audits/inspections on subcontractors shall be raised, discussed, and resolved with the relevant contractor (with whom the subcontractor is contracted). 5

LETTER OF GOOD STANDING The Principal Contractor shall submit a letter of good standing with the Compensation Commissioner or Insurer at the tender stage. The letter of good standing shall be valid throughout the project period. 6

SHE PLAN The Principal Contractor shall develop, compile and issue a SHE Plan as per the Eskom SHE specifications. The SHE Plan shall be submitted for approval to the Eskom prior site establishment. For ease of reference, the format and minimum contents of a SHE Plan is attached in Annexure-1. 7

APPOINTMENTS The contractor shall provide an organogram listing the legal appointees and shall ensure that the organogram is updated as and when appointments change. The contractor shall ensure that all his/her appointees are made aware of their accountabilities and responsibilities in terms of their appointment and that he/she advises and assists these appointees in the execution of their duties. Copies of any appointments made by the contractor shall be included in the health and safety plan and be provided to the client/agent. 9

APPOINTMENT NOTE: the actual appointments shall be communicated to the health and safety manager on site and shall include a copy of the appointment letter, a curriculum vitae, and relevant certificates of competence for the proposed role. Depending on the nature of the contract, the assignations will be expanded. The Principal Contractor shall compile and maintain a current register/inventory of all supervisory and management personnel appointed for the contract. The inventory shall include SHE and professional competencies for each person appointed. The inventory shall be submitted to the client for approval preferably at tender stage as part of the SHE plan. Supervisor must at least have working experience in the field of “Meter reading, disconnecting and reconnecting 11

RISK ASSESSMENT The Contractor shall ensure that a detailed HIRA is conducted a competent person before commencement of Meter reading, disconnecting and reconnecting work and it shall form part of the health and safety plan to be submitted to the client for approval. The Contractor shall be responsible for making sure that all employees under his control are conversant with the content of the HIRA and what appropriate measures have been put in place to either eliminate or reduce the risks. On site risk assessments shall be conducted before any work can commence 12

SAFE WORK PROCEDURE The contractor shall develop safe working procedures for high risks tasks. The contractor shall make the employees to be conversant with SWP and ensure that tasks are carried out according to the SWP. 13

SAFETY, HEALTH AND ENVIRONMENTAL TRAINING The Principal Contractor shall conduct a training needs analysis on receipt of the letter of appointment from the Client. The training conducted shall be three fold, namely: Induction – the Principal Contractor shall ensure that all his employees undergo a SHE induction based on the legislative requirements, SHE Plan and HIRA before starting work on site. Awareness – the Principal Contractor shall continuously conduct on site toolbox talks, display posters and discuss bulletins and case studies from the client. Competency – the Principal Contractor shall, based on his HIRA and training needs analysis nominate relevant persons to be sent out on appropriate courses. The Principal Contractor shall make available at the Client’s request copies if certificate of training. 14

REPORTING, RECORDING AND INVESTIGATION OF INVESTIGATION OF ACCIDENT AND INCIDENTS All accidents and incidents must be reported, recorded and investigated in accordance with OHSA and other applicable environmental legislation. The Principal Contractor shall report all accidents and incidents to the Client within 24 hours. The Principal Contractor shall ensure that an Eskom representative is invited to any investigation linked to an activity within the scope of the project. The Principal Contractor shall keep on site record of all incidents reported in the form of OHSA GAR Annexure-1 and submit copies to the Eskom. In case of accidents/incidents, Eskom may conduct an independent investigation. 15

INJURY STATISTICS The Principal Contractor shall report monthly injury statistics to the Client or his agent in the following manner:  Number of fatal injuries  Number of lost time injuries  Number of medical injuries  Number of first aid injuries  Number of motor vehicle accidents  Number of property damage incidents  Number of environmental incidents  Number of public incidents  Number of electrical contacts  Number of near misses  Man hours worked 16

EMERGENCY PREPAREDNESS The Principal Contractor must develop and submit to Client an emergency response plan from a review of potential emergency scenarios before commencement on site which will include, but not be limited to: Employee accounting systems after emergency  Rescue and medical duties  Means and procedures for reporting emergencies  Emergency Contact list The emergency plan shall include training of employees to deal with such emergencies and shall form part of the SHE plan. The Principal Contractor shall advise the Client or his agent within 24 hours of any site emergency occurring together with the action taken. 17

TRANSPORT The Contractor shall ensure that all road vehicles used on the premises are in a roadworthy condition, are licensed, and are insured. Vehicles should have an identification stating that the Contractor is an Eskom Contractor. The All drivers shall have relevant valid driver’s licences, and no vehicle shall carry passengers unless it is specifically designed to do so. All drivers shall adhere to the speed limits and road signs on the premises. It is further expressed that contractors should ensure Zero Harm to employees while performing their duties and even travelling. All vehicles are provided with a serviced portable fire extinguisher and a vehicle first aid kit at all times. 18

PERSONAL PROTECTIVE EQUIPMENT (PPE) The Principal Contractor shall, based on his HIRA, put measures in place to prevent the employees from dogs, snake & insects bites and bee stings; and must ensure that the necessary & relevant medication is always readily available to the field workers at all times Also, the Principal Contractor shall, based on its HIRA, establish what type of PPE should be worn by his employees based on the nature of hazards and the type, range and performance of the protection required. The Principal Contractor shall ensure that all employees are trained in the correct use, maintenance, care and the limitations of PPE. The Principal Contractor shall ensure that the requisite PPE is worn at all times. The following are some of the PPE which must be worn and used:  LV gloves  Face-shield  Safety booths  Dazer  Long pants and top 19

MONITORING AND EVALUATION The Principal Contractor shall carry out inspections as prescribed by legislation, his SHE Plan and Client SHE specification. The Principal Contractor shall keep records of all inspections. 20
