Tailor made Wound Management in a General Practitioners Lead Wound Expert Center Maartje Hesseling, Lupine Alphen, The Netherlands Supported with a scientific grant from Lohmann & Rauscher EWMA London 2015: EP ID1

Aim of the Project Enable effective leg ulcer management and care for other wound types in a specialised wound clinic as part of a general practitioners community center. Case ascertainment was used and the results of typical patients treated at the center are reported. EWMA London 2015: EP ID2

Methods - Activities Training of the staff on approaches to leg ulcer management and compression Adress specific needs: Elderly, frail patients who cannot tolerate compression bandages Those who remove them due to mental status etc. Adress specific needs: Elderly, frail patients who cannot tolerate compression bandages Those who remove them due to mental status etc. Prevention project: Screening of N = 577 patients at risk for venous hypertension or those that had a venous leg ulcer previously After assessment N = 254 received further vascular tests. Of these patients N = 136 received compression stockings and are followed up with once a year visits and renewal of their compression stockings. Prevention project: Screening of N = 577 patients at risk for venous hypertension or those that had a venous leg ulcer previously After assessment N = 254 received further vascular tests. Of these patients N = 136 received compression stockings and are followed up with once a year visits and renewal of their compression stockings. Started 2 years ago and is ongoing EWMA London 2015: EP ID3

Results / Discussion Venous leg ulcer patients received a rigid compression system of reusable bandages** or depending on the amount of oedema a 2-layer compression bandage system with a cohesive*** bandage as a top layer. Patients with lacerations and chronic venous hypertension were succesfully treated with a moist wound healing dressing and a tubular* 2-layer compression system. *Actico Silk, **Elco compression, ***Rosidal TCS, Lohmann & Rauscher EWMA London 2015: EP ID4

Conclusion Tailor made wound management is succesfully delivered in the community. Patients reported to be satisfied with the care they have received and apreciate not to have to go far for optimal care. EWMA London 2015: EP ID5
