DEVELOPING & EVALUATING OPTIONS What do we mean by an ‘option’? The focus of FutureFit is: To agree the best model of care for excellent and sustainable acute and community hospital services that meet the needs of the urban and rural communities in Shropshire, Telford and Wrekin, and Mid Wales. The Clinical Model sets out the overall shape and character of the system for local health care. We now need to set out ways on which current acute and community hospital services could be delivered under the new model. There are a number of key components to this: A single Emergency Centre Some Urgent Care Centres A single Diagnostic & Treatment Centre Facilities for Local Planned Care appointments Some community Health Hubs. There is an infinite number of ways in which these components could be configured locally. Each of these ways is potentially an ‘option’ – a unique combination of the number, location and co-location of the model’s components.

DEMOGRAPHIC MAPS INTRODUCTION In preparation for the evaluation workshops on Tuesday 17 June, the following maps and charts have been prepared to inform the development of options. MAP INFORMATION Super Output Areas (SOAs) Each map is split into small geographic areas known as ‘Lower Super Output Areas’ (LSOAs). These are official government statistical areas and cover all of England and Wales. They all represent approximately 1,500 people and the areas therefore vary in size according to population density. Other information included on the maps Each map includes motorways, ‘A’ roads, railway lines and the main towns and villages. Where possible, the maps show population information for Shropshire, Telford & Wrekin and the eastern side of Powys.

POPULATION & ACTIVITY CHARTS CHART INFORMATION CHART 1: Population Pyramid This chart shows population for Shropshire and Telford & Wrekin now (2013) and projected forward to The ‘x’ axis indicates the size of the population in thousands and this is for single year groups (i.e. 1, 2, 3 etc.) as indicated by the ‘y’ axis. The information is shown for females and males. For example, there are slightly fewer than 3,000 males aged up to one and about 3,300 males aged 28. CHART 2: Unplanned Acute Utilisation Rates and CHART 3: Planned Inpatient Utilisation Rates These two charts show the number of emergency hospital admissions (unplanned acute utilisation rates by single year of age) and pre-booked hospital appointments (planned inpatient utilisation rates). Both charts show this information for males and females and by average activity per person for single year groups (i.e. 1, 2, 3 etc.).

MAP 1: Population Density The number of people living per hectare is shown using five categories, with the darkest colour representing the most densely populated areas. MAPS 2, 3, 3 and 4: Age Maps (0-15 / / / 65+) These maps show the numbers of people in these age groups across Shropshire, Telford & Wrekin and part of Powys. The information is based on the figures from the 2011 census. MAP 5: Benefit Claimants of Working Age This map shows the total number of the population within each LSOA who are claiming one or more benefits from the Department of Work and Pensions and who are of working age (16-64). The information is based on the latest figures from November MAP 6: Pension Credit Recipients This map shows the total number of the population within each of the LSOAs who receive a pension credit. The information is based on the latest figures from November 2013.

DEMOGRAPHIC MAPS MAP 7: Index of Multiple Deprivation The Index of Multiple Deprivation (IMD) is an official indicator to give an overview of a population’s social and economic health in one statistic. The IMD is calculated using seven domains and 38 pieces of data covering: Income Employment Health deprivation and disability Education skills and training Barriers to housing and services Crime Living environment The IMD was last calculated in 2010 and some of the data used is from The next IMD will be released in Equivalent information for Wales is not available.
