What WellTrackONE Does We assist physicians and healthcare organizations generate significant revenue and predict risk. Sending postcards out to all eligible Medicare patients Call Medicare patients and scheduling them into allocated appointment slots Train the nurse, CMA or mid-level to complete the AWV in the exam room Quality controlling the data Produce the Medicare-approved report for the patient’s chart Send the reports and the data back to the practice for their EMR/EHR Give physicians and care coordinators access to the risk prediction scores, HCC RAF Scores and trending outcomes data Repeating this process every year

Effectiveness of Postcards WellTrackONE will send out postcards to all of your eligible patients. It’s very important to communicate with this population by mail since are majority of Medicare patients are not ing or texting: Postcards are customized to your practice with your logos and branding They will be sent to all your eligible patients WellTrackONE pays for the printing and postage, there is NO COST to you

We Call Your Patients After the patient receives the postcard, we call them. We make sure they received the postcard and then we schedule them into allocated appointments at your practice for their AWV: We will be representing your practice when we speak to your patients We will book them into appointment slots your practice as allocated for this program We will explain to the patient what the Annual Wellness Visit is, how it is different from a physical, etc. We will collect some preliminary information from the patient including their current providers of care and what pharmacy they use We also explain to the patient that they are not meeting with the physician but a member of the physicians team who is trained to perform the AWV. Medicare allows a medical professional such as a CMA, LPN or RN to perform this informational gathering session

Annual Wellness Visit Template The AWV starts with collecting the patients vitals, including their O2 Saturation and waist size Next we ask questions about the patients diet We then move through the AWV collecting over 280 data points from the patient. As we get towards the end of the AWV, we do a Safety analysis, MCI/Cognitive Assessment, and Depression & Anxiety Analysis using the PHQ-9 scale We finish this 30 – 45 minute visit with Observations: Simply questions that will give the physician a greater in-depth view into the patients health and life When the AWV is complete, the medical professional will select the Save button

Clinical Triggers™ WellTrackONE’s proprietary Clinical Triggers provides your healthcare organizations with point-of-care assessments from data collected from the patient and produces medically necessary treatment options. They are rapid evaluations of all the data that has been collected during a Medicare Annual Wellness Visit and show the nurse/caregiver what treatment recommendations are pertinent to these evaluations.

Provider Report – Summary of AWV & Required Sections This is the report that the physician receives. It has been vetted and approved by Medicare in 2011 It has all the components required by Medicare and will survive any audit by CMS This Report summarizes all the data points collected during the AWV

Patient Report WellTrackONE provides the Patient Report for you to print and present to the patient This report is a required section of the Annual Wellness Visit We make this report available in English, Spanish and about 5 other languages

Reportal™ – A Population Management Tool All the data collected during the AWV is incredibly valuable for your organization. We give you this valuable “Reportal” tool that lets you download your patient data into an Excel spreadsheet or print it out on the screen as trend reports. These reports risk stratifies your patient population so your care team can manage the patient population. The Reportal is incredibly valuable for clinical measures, outcomes, HEDIS reports, ACO reports, PCMH measures, Medicare Advantage plan requirements and other necessary population management requirements.

Downloadable Data via our CCD Extractor Finally, we give you a program to download our data into a Continuity of Care Document (CCD) format to import into your EHR.
