Shared/user docs/general/guidelines on photo permission form Staff guidelines on completing the photo permission form Form A – For those able to give consent:- Ask each person what the form might be about – if they are not sure, ask what the photos are of and build on that Ask if they are happy or not for their photos to be used in each of the 5 ways listed. Chat about what this means and check with open-ended questions (ones that need more than a ‘yes/no’ answer) that the person understands. Use the final ‘thumbs up’ / ‘thumbs down’ boxes to check that the person knows what they are agreeing to If extra help is needed: Show images of the different choices of how we might use images (see attached for help) Explain what the result of using their pictures in different ways or not using their pictures might be (see attached sheets for help) Use whatever communication method you know works with that person to help them understand and to check they know what they are signing for When you are happy the person understands what they are signing, make sure you could tell someone else how you decided they understood. If you use contact sheets, you might want to make a note of the decision on that person’s contact sheet

Shared/user docs/general/guidelines on photo permission form Form B – for those unable to give consent:- If you have tried a range of appropriate ways to help the person understand the photo permission form and they are not able to: understand the information about the decision retain that information use the information to make a decision communicate their decision Then you are able to make a ‘best interest’ decision on their behalf, taking into account what you and others who work with you know about the person and any choices that person may have expressed at another time about being photographed. You need to confirm in writing why it is in the person’s best interest to be photographed/videoed – the list on the ‘Best Interest Photo/Video Form’ will help you.
