Multiple students at a site? How many forms? 25 If students are doing same work tasks, or are together on one tour, then one completed form is acceptable. Leave the student name line blank, and copy for each student to have for parental signature. If grouped by a department or a role, then one completed form for each department or role is acceptable. Again, leave the student name line blank, and copy for each student to have for parental signature. Do not copy risk assessment content from term to term, or year to year. New placements, or new dept.’s/roles with an existing placement, need a new risk assessment. This is because tasks and supervisors change, and every student brings something new to the placement. Send questions about student readiness to John or Board CBL consultants !

29 Risk Assessment Forms: Records Keeping 1. EECD and school boards require it 2. Used for regular and emergency contacts 3. Provides a record that policy was followed 4. Used as a reminder for what all parties agreed to 5. Provides helpful reference and contact information 6. There as a learning resource for the teacher. We can learn something new with every risk assessment, and keeping them is a great way to look back before starting a new one!
