Brookside is a state of the art Extra Care Housing scheme that opened on 16 th April 2012 with integrated Health, Well-being and community services, Enhanced Day Care and services to people with Dementia and their carers. Brookside is the largest Department of Health Extra Care Housing Fund grant allocation of the national bidding round, delivering 111 high quality self contained 1 and 2 bedroom apartments. What is extra care? designed to create a community where people can live an independent life in a socially supportive environment suitable for people who want to live in their own home, but have access to care and support services, if required self contained accommodation with communal facilities designed to enable people to engage in a full and active social life at a pace they are comfortable with should care and support be required it will be tailored to meet individual needs specifically designed to offer security, easy accessibility and there is a emergency call system in every apartment in case of an emergency or fall, monitored 24/7.

Who can live in extra care? We aim our services at older people over 55, with a clear need for the care and support services that will be provided We understand and appreciate that those with early stages of dementia can benefit and we consider that others may develop dementia over time. Care and support will sustain independence as long as is appropriate. Priority is given to people who already live in West Lancashire, although people who have an existing or former association with the borough can also be considered.

Revenue Funding Rents are LA target, enhanced for Extra Care (£ bed, £ bed). Service charges add £54.01 p.w. Leaseholders maintenance £12 p.w. Utility/Amenity bills are £18.91p.w. ** All charges bar personal utility charges have been agreed as HB eligible. We have a ‘Shared Facilities Agreement’ with the AHA who pay a substantial 3 figure sum on an agreed formula, to offset the service charge to residents. We have tendered the Bistro and Hairdressing services and both pay rental which will be credited to the service charge.

Dementia Provision - Design Day centre Spaces with meaning, including a dining room, activity rooms, lounge and assisted bathing room. Colour specific – e.g. Yellow used in dining room to encourage eating. Dementia friendly garden/walk way Occupational therapists help with suitable furniture.

Assistive technology Telecare Tynetec warden call system – ability to add reminders. Pendants programmed for all units. Full range of radio linked telecare peripherals can be linked (eg bed sensor, light sensors, fall detectors, flood detection). Telehealth potential. ‘Just checking’ – 5 acquired to aid service user need assessment, can be moved as required. ‘Memaxi’ – 10 units acquired, to allocate to users to facilitate independent management of daily memory tasks and living.

A further £9.75 p.w available for assessed and agreed enhanced needs. Care Funding via personal budget to FACs assessed service users with substantial or critical needs.

Allocation into the service Allocation panel that meets on an ongoing basis to allocate the properties in accordance with the model All applications are processed by West Lancs and bought to panel Every effort is made to ensure a balance of community whilst meeting the allocation criteria Allocation criteria was agreed early stage of the development to ensure an understanding across those disciplines involved with the process Referrals into the service are key to the success of balancing the community whilst fulfilling the contracts in place
