Madison County Office on Aging 2015 CSBG Needs Assessment Survey The Community Services Block Grant (CSBG) customer needs assessment survey was mailed to past and present CSBG customers asking them to identify specific needs for each of following categories:  Child Care & Child Development  Parenting & Child Support  Transportation  Health & Basic Needs CSBG | Madison County Community DevelopmentKyle J. Bushaw 4/19/2016  Employment  Education  Financial & Legal Issues  Housing, Food, & Nutrition CSBG completed 124 customer surveys, 55 of which the respondents were aged 55 or older: Male (N=14), Female (N=41). 1

Madison County Office on Aging 2015 CSBG Needs Assessment Survey A community-wide Needs Assessment survey was also distributed to local elected government officials, social service agencies and other community leaders/citizens for feedback to identify community needs and priorities for low-income families, households and neighborhoods. CSBG completed 270 community leader surveys. Respondents were largely made up of property owners (63%), Social Service Organizations (18%), and municipalities, villages, and townships (16%).
