Background

Country profile The Republic of Suriname is located at the north-east coast of South-America Between French Guiana in the east, Guyana in the west, Brazil in the south and the Atlantic Ocean in the North Occupying an area of about 163,830 km 2. With slightly more than inhabitants

Population at risk The population at risk ca comprises of two different groups: the stable village population and the mobile/unstable population. The populated interior can be stratified in three main areas Amerindians villages Maroon villages Garimpos, gold mining areas

Malaria control in Suriname

National coordination structure The Ministry of Health (MOH) in Surinam is responsible for all actions related to malaria. A national malaria board function as the main technical advisory group for the policy making process.

History of malaria control in Suriname Beginning of the 20 th century focus on case management and chemoprophylaxis for travelers From 1949 to 1955: larval control and residual insecticide spraying as main strategies. In 1958: MEP adopted. Transformation of the Insect Control Program into the MEP accordingly.

History continued The MEP focused on the following strategies: Vector control: Active Case Detection: Case management: The MEP resulted in the elimination of malaria from the coastal area in Suriname The MEP did not succeed in eliminating malaria from the interior

History continued In 1984: the operational malaria control activities in the interior were transferred to Medical Mission Primary Health Care Suriname (MMPHS) The Medical Mission PHCS has 56 health post in the interior staffed with 105 healthcare assistants and 23 microscopist In 1997: the MM introduced ITN for all villages in the interior

The National Malaria Board In 1995 the National Malaria Board was installed The policy of malaria control and prevention in Suriname has been translated in four main strategies: 1.Free Rapid diagnosis and prompt effective treatment. 2.Free Distribution of LLINs to the population in areas of risk. 3.Behavior Change Communication (BCC/IEC) 4.Selective vector control (with emphasis on the use of LLINs).

Preparations for LLINs Distribution

Consent building and community support Meetings were held with tribal leaders to gain consent and support Consultations were held to re-design nets to meet the expectations of target populations

KAP studies A baseline MIS was conducted in 2006 in stable communities and in 2009 in the mobile communities Key findings for stable communities were as follows: About 70% of both groups had ever used a net and most people knew where to obtain a net 75% of the Amerindians were using a mosquito net to prevent malaria < 50% of the Maroons were using a mosquito net

KAP studies continued Key findings in the gold miner population: 59.2% knew that a bed net was to protect oneself against malaria 47.4% use a bed nets to protect themselves against mosquitoes 59.4 % of the respondents had slept with a bed net the previous night The main reason mentioned to not use a bed net is not having one.

Forecasting and Procurement The MM registration database was used to forecast the number of nets needed A census was done in gold mining areas Procurement was done by the PEU for MM Malaria Program Procurement was done by the procurement department of the MOH for the gold miners program

Distribution of LLINs Transportation to the interior by car, airplane and boat (ATV in gold mining areas) The already existing logistics of the Medical Mission PHCS was used for the distribution For gold mining areas sentinel sites were selected to function as intermediate storage People had to identify themselves with the MM registration card Gold miners did not have to identify themselves

Coverage reached The Medical Mission Malaria Program distributed about from 2006 – 2008 Reaching over 90 % coverage of the target population The MoH Malaria Program distributed in 2010 Reaching 94% coverage of estimated gold miners population

Monitoring & Evaluation

M & E surveys After 3 months of distribution M & E surveys were conducting Very low percentage of usage revealed 12% Color of the nets Privacy issues Led to locally designed combination bed net and replacement of previous LLINs Surveys conducted in 2010 in stable communities revealed 60% - 91% use

Lessons Learned Understanding the customs of mosquito net use is essential in the successful implementation of LLIN as malaria control strategy

Impact of different control strategies

Malaria transmission in Suriname has decreased significantly over the past seven years, a period in which important and large-scale integrated control activities took place, including the mass-distribution of Lon Lasting Impregnated Nets (LLINs). a dramatic decline in the number of malaria cases a significant decrease in hospitalization a tremendous decrease in mortality a significant decrease in the An. darlingi population
