3 3 State of the Science Objectives: define existing evidence used to guide nurses and social workers in supporting family caregivers of older adults Identify competencies needed to support family caregivers, and methods to develop them Identify promising nursing and social work interventions to assist caregivers, as well as strategies to increase the ability of nurses and social workers to provide support to caregivers

4 5 Invitational State of the Science Symposium 2008 interdisciplinary symposium of 50 RNs, SWs, family caregiver researchers, and others Presentations of peer reviewed papers summarizing the “state of the science” Recommendations from the symposium circulated to all participants for feedback Report published as supplement to Sept AJN and as special issue of JSWE

5 8 Focusing on a “Family-Friendly” Paradigm In a joint press release, the partners pledged to collaborate with families in new ways to: Improve families’ ability to better manage everyday care responsibilities Reduce caregivers’ own burdens and health risks Promote better quality for both the older adults receiving care and the family members providing it

7 3 Why New Jersey? History of caregiver support programs - state funded since 1980’s Tradition of using federal initiatives to shape system development efforts in state Process of developing ADRC’s in every AAA in the state Saw a need to integrate caregivers within all home and community based services shifting the paradigm to person and family centered services

8 9 – The goal of the Pilot is to improve caregiver knowledge and skills through access to information, training and services for informal caregivers of those adults receiving HCBS waiver services. Pilot includes: brief caregiver questionnaire (context, direct care, troublesome behaviors, self-care, informal supports) tailored information and referral to services two follow up calls from Care Manager Pilot design recognizes the constrained resource environment facing state and local services Builds from the policy recommendation that caregivers should be integrated into health and social service systems - particularly if those systems rely on informal caregivers to provide LTC New Jersey Care Partners Support Pilot

9 11 How Does it Work? Self-administered questionnaire at time of care plan, monthly visit or yearly reassessment Caregiver Action Plan - quick scan and check boxes Four areas to query: direct care of relative/friend; management of troublesome behaviors; caregiver self-care; increasing informal supports Provided to caregiver: information and referral module(s) related to the four topic areas; brief resource listing; article(s) on identified topics Care Manager is to provide encouragement, information, recognition and to check back in with the caregiver over next two months

10 How Does it Work? – Staff receive 6 hours of training on caregiver assessment and needs; caregiver questionnaire and how to use the results to tailor a response; protocols of the evaluation component – All materials to be used by the Care Managers have been developed or assembled (articles used by permission) by Family Caregiver Alliance – Expectation that pilot will have 130 caregivers enrolled within 3+ months and the pilot test will take place in 4 counties: Warren, Mercer, Monmouth, Atlantic 13

11 5 Pilot Evaluation – The Hilltop Institute, University of Maryland, Baltimore County will conduct the evaluation – Evaluation will focus two areas: – Outcomes: whether caregivers used services, attended trainings/support groups and improved skills/coping with relative/friend; – Process: care managers pre/post knowledge levels re: caregiver assessment/needs; materials/process for caregiver interactions – Telephone interviews with FCA, ADRC Directors

12 10 Pilot Timelines Training for Care Managers mid-February Pilot to begin enrolling caregivers March 1, 2011 Technical assistance and conference calls to Care Managers on materials, process Month 1 and 2 Evaluation questionnaires to caregivers 3 months post-enrollment Survey to Care Managers at 6 months Evaluation to be completed by end of 2011
