Date & Time: 14:30 on Feb 12(Monday), 2007 Place: Factory water supply and drainage worksite Victim: 51-year-old male painter for OO Paint company, subcontractor He was climbing the ladder leaning against the mobile scaffold to prepare the painting work on the upper part of the side wall (S/W panel) of the factory warehouse building. He died as a result of fall down to the asphalt concrete ground from the untied ladder (approx. 2.5m high) as the mobile scaffold moved around because of unlocked casters. Fatality Overview Site Overview Factory water supply and drainage worksite - Interior & exterior wall painting and floor epoxy work was being performed for factory, warehouse, and office building (90% of total processing rate)

◎ The victim and 3 co-workers arrived at the site and performed the painting work for the upper exterior wall (S/W panel) of the factory building until around 12:30. ※ Factory & warehouse building: R.C.+ S/W Panel (S/W panel for the upper exterior wall) ※ One worker performed the painting task (oil paint + thinner) on the mobile scaffold(2.5m high, mobile scaffold + each pipe welded) for the exterior wall with the spray equipment, while 2 workers conducted the auxiliary work on the ground. After spray painting is complete, one worker performed the painting job of the building corner with a brush. ◎ Workers moved the mobile scaffold near the north wall of the warehouse building and partly performed the spray painting on the upper part of the exterior wall until around. They took a rest after they moved the mobile scaffold to the left for the next spray painting work. Fall from Ladder while Climbing to the Mobile Scaffold 3. Fatality Description

◎ Victim was climbing the ladder leaning against the mobile scaffold with the caster locking device unlocked in order to perform the preparation work such as the wall cleaning on the north side of the building before the spray painting, in the space between the building wall and the mobile scaffold at around 14:30, incident time. - He died as a result of fall from the ladder down to the asphalt concrete ground. ※ The ladder was vertically installed in between the north wall of the building and the mobile scaffold. ※ Victim was bleeding from his mouth without hard hat on. ◎ Unlocked caster locking device and the untied ladder - Casters for the mobile scaffold was not locked, so the mobile scaffold moved around while the victim was climbing the ladder. - The ladder was not tied to the mobile scaffold. Victim died as a result of fall (approx. 2.5m) from the ladder. ◎ Personal Protective Equipment Not Worn - Hard hat must be worn at the elevation work using the mobile scaffold. ◎ Lock the casters of mobile scaffold and the ladder - Mobile scaffold casters must be locked and the ladder be securely attached to the mobile scaffold. ◎ Wear Personal Protective Equipment - Personal Protective Equipment such as Hard Hat must be worn at elevation work.
