Introduction and Methodology Overview This edition quantifies the distribution of 2015 prescription volume in each state between Medicaid, Medicare Part D, and all other payers. Total CY2015 prescription volume was obtained from the Kaiser Family Foundation ( Medicaid prescription volume was tabulated using quarterly CMS data for FFY2015. These data include Medicaid prescriptions paid in the fee-for-service setting and those paid by Medicaid managed care organizations. Data on Medicare Part D prescription volume by state was tabulated using another CMS dataset for CY2013; this volume was increased by 10% to estimate CY2015 Part D prescription volume. Note that roughly 25% of Medicare beneficiaries do not participate in Part D, and the Part D prescription volume we have tabulated does not include mail order. Prescriptions for all other persons – those with commercial insurance, non-Part D Medicare, health insurance exchanges, persons with no insurance, etc. – are derived by subtracting Medicaid and Medicare Part D figures from each state’s total prescription volume. 1

Summary Observations Medicaid represented 16% of national prescription volume during 2015, with its share of total prescriptions at the state level ranging from a low of 7% in South Dakota and Wyoming to a high of 27% in New York. Medicaid represented 42% of prescriptions filled in the District of Columbia. Medicare Part D represented 36% of nationwide prescriptions during At the state level, the Part D program’s share of total prescriptions ranged from 24% (in Utah and Wyoming) to 53% in North Carolina. Commercial and other payers accounted for just under half (47%) of total national prescription volume during At the state level, this percentage ranged from 31% in Rhode Island to 69% in Wyoming. This “other payer” category represented only 19% of prescriptions in the District of Columbia. 4
