 A need to have a method to record time spent in Public Health  1991 – Billing added to Champ (all paper)  1996 – Public Health Agencies using Champ desired public health and home care documentation with Champ explored documentation methods.  – System development and Omaha System chosen based on its framework of;  Problems, categories, and outcomes

 1997 DVHHS commits to Champ for PH Documentation  1997 all staff attend Omaha System Training in preparation for electronic Champ rollout  January 1998 all staff of agency Public Health and Home Health Care begin using 100% of the clinical features of Champ. ◦ Various methods used over the course of time; desktops, PDA’s, notebooks and laptops.

 1998 – 2010 DVHHS builds pathways in Champ based on the Omaha System.  2011 NightingaleNotes was developed, incorporating Omaha System.  2012 Using NightingaleNotes moving to EHR

 In 2014, Minnesota became the first state to recommend ANA- recognized point-of-care terminologies be used in all Electronic Health Records (EHRs).). While MN is the 1st and only state to date with that standard, ANA has recommended that others do the same.  The Minnesota Department of Health recognizes that Omaha System should be used by community providers of care such as Public Health, Human Services and Accountable Care Organizations.

 We are in alignment with national requirements for standardization of data and information exchange  DVHHS use Omaha System and NightingaleNotes

 2013 – Connectivity Grants in MN, grant dollars to develop Continuity of Care Documents for Electronic Record Exchange through a MN Certified Exchange.

. DVHHS is a member of the first Accountable Care Organization ( ACO) in Minnesota and the nation Formed by 12 MN Counties the ACO of which is named Southern Prairie Community Care (SPCC).

 SPCC is self governed through a Joint Powers of 12 counties.  Administrative team; Director, Medical Director, Integration Specialists and contracted consultants.  Partners; Human Service Agencies, Public Health Agencies, Mental Health providers, Hospitals, Clinics, University of Minnesota Extension and Community Health Workers

 SPCC have partnered to develop The Triple Aim for clients served in our area that are the highest cost & most frequent clients in Emergency Departments.  SPCC and DVHHS are both Omaha System users of NightingaleNotes.

 Currently through SPCC and the work of Champ Software DVHHS is capable of electronic health exchange.  SPCC developed Consent for Exchange  Nightly through a Champ portal uploading clients to the Sandlot Exchange.

 We can exchange with SPCC members.  Sole Purpose- To Achieve the TRIPLE AIM for clients we serve.
