Naviance forms Complete the Naviance forms by June 17 th. DIRECTIONS log into Naviance click “about me”, on the left-hand side of page see “surveys to take” Parent information form Counselor information form Teacher information form 1 and 2

Teacher Recommendations First step: 1. Identify a teacher. Does not need to be a class that you have an “A”. Prefer 11 th grade academic teacher No coaches, advisors, mentors, supervisors Second step: 1. Speak to the teacher. Ask teacher. Follow up with teacher when necessary paperwork completed.

Waiver of Right to Access Letters of Recommendation Submitted with Transcript Request Form 20 School Days Prior to First Application Deadline Letters of Recommendations WILL NOT be sent if this form is not completed and rights are not waived

TRANSCRIPT REQUEST FORM Submit 20 School Days Prior to First Application Deadline *Use Chart Students MUST meet in- person with a Registrar to submit their transcript request form Requests 1-3 no charge Requests 4+ ($2 per transcript)

Teacher Recommendation Request Does your school have a stamp icon? If yes, provide your teacher with an addressed, stamped envelope so they can mail your recommendation. IMPORTANT! A new form will need to be completed and given to teachers if you add more schools at a later date!

NCAA Eligibility Center DI/DII College Athletics 1.This should be done by the end of junior year. 2.NCAA must be added to your Transcript Request form 3.SAT/ACT must be sent to NCAA
