Self-Management Programs Chronic Disease Self-Management Program CDSMP Chronic Pain Self-Management Program CPSMP

Chronic Disease Self-Management Program The CDSMP was first developed in 1991 by Stanford University and Northern California Kaiser Permanente Medical Care Program.

Chronic Pain Self-Management Program The CPSMP was developed in the late 1990’s by Dr. Sandra LeFort of McGill University in Montreal in conjunction with Dr. Kate Lorig of Stanford University.

Underlying Assumptions People with mental and physical chronic conditions and/or chronic pain have similar concerns and problems. These people must deal with not only their specific conditions/diseases or pain, but also with the impact the condition or pain has on their lives and emotions.

Lay people with chronic conditions, when given a detailed leader’s manual, are able to teach self-management education as effectively as health professionals. The process in which the CDSMP and the CPSMP are taught is as important, if not more important, than the subject matter that is taught.

Workshop Basics Each workshop consists of six sessions. Each session is 2 ½ hours. Each session has 5 to 8 activities. Each activity has specified time limits. The methods for presenting each activity are specific. The programs are standardized and MUST be presented as written.

Participant Involvement Designed to involve the participants. Emphasizes ways to increase confidence. Structure and support usually leads to good health decisions. Best learned in little bits, try things out, get feedback, and build on what learned.
