WELCOME! Dear ChaplaincyPlus member Welcome to our Discipleship Needs Assessment Exercise 2016! This exercise is designed to assess your opinions and preferences on one overall topic: “discipleship in the workplace”. The questionnaire takes quite a unique form. It is a Powerpoint deck which you actually edit in order to provide your answers. So keep it in “normal view” and work your way down the slides to complete it. The next slides (3-8) will walk you through what we want you to do.

2 Types of Question This questionnaire contains two types of question. Both are attempting to understand a “relative preference” from you. There is no right answer! You’ll see slides that look like this And others that look like this

2 Types of Question 1. These Venn diagram slides offer three intersecting ideas about the topic at the head of the slide 2. Each circle contains a statement and a text box with a ‘%’ symbol already in it 3. We want you to type a number from in each text box

100 % The object of this exercise is to distribute a maximum budget of 100% across all three boxes. This should reflect the proportion with which you agree with the unique statement in the circle… You do not have to use up the whole 100%, just think of it as an “agreement budget”… You can use the whole 100% on one statement, if you wish. You can set any, or all, statements to 0% agreement, if you wish. But don’t move the boxes into the intersections, that’s cheating, we need unique agreement!

2 Types of Question Every other question is a slide with three statements in it like this one. We are forcing you to make choices between these statements. Show your “choice” by changing the colour of the text. 1. Take the statement you agree with the least and set the font to red. 2. Of the other two statements take the one you agree with the most and set the font to green. 2. If you TOTALLY disagree with any statement, simply delete it (that might be all of them).

HUGELY IMPORTANT! When you get to the end of your questionnaire, SAVE the file. your completed questionnaire to: All responses are treated as confidential and all reporting of results will be completely anonymous.
