Copper & Coal HSEC Alert DISPLAY ON NOTICE BOARDS FOR 30 DAYS AFTER INCIDENT Preliminary Classification: Loss Time Injury - LTI. (Incident ) Operation / Project: RTKC Mine Operations – Roads & Projects Location: South East Waste Rock – A4 Section Date: January 19, 2016 Critical Control Absent: Contact: Amit Walia What happened: At approximately 10:40am, a Mayday was initiated in response to the south east waste rock expansion A4 area. A mine operations employee was in the area conducting an inspection for the future establishment of dump catchment berms. Interdepartmental evaluation around site exposure for walking on uneven surfaces.  People: The employee was assessed by lark EMT and transported off site by county ambulance to the local hospital for further evaluation. Preliminary causes:  Environment: Loose rock, uneven surfaces, ground conditions.  Equipment: Proper PPE was utilized (hard hat, safety glasses, good boot condition).  People: Employee loses footing on windrow and descends uncontrolled resulting in fracture to right tibia.
