Date & Time: 17:00, March 21, 2007 (Wednesday) Place: new construction (complex building) site Victim: a worker of a subcontractor for plastering work (male/45, Chinese Korean) While the victim was dismantling steel pipes installed to support scaffold and footing installed for wall polishing work on upper part of the 1 st basement ramp in dry area, He lost his balance and fell down to ramp floor below 10.5m along with scaffold steel pipes to death. Falling while dismantling scaffold and footing 1. Fatality Overview Site Overview New construction site of a combined building of steel-frame and reinforced concrete structure (with 20 stories above ground and 5 below) - Reinforced concrete frame on the fifth ~ eighth floor above ground and steel frame assembling on the 12 th ~ 20 th floors above ground were completed. (overall process rate was 45%; plastering work overall process rate was 38%)

The victim arrived at the site with other two co-workers at about 06:30, received a work order from a supervisor, and then started pillar and wall polishing work. The victim started wall polishing work in a fan room from 07:10 and the other two co-workers conducted wall polishing work on the upper part of first basement ramp in dry area (4 dry areas including accident spot). At about 15:40, the victim finished wall polishing works in a fan room and had lunch. After lunch, the victim had dismantled footing and steel pipes in the upper part steel out of pipes and footings (2 bucks high footing installed for polishing work) with two co-workers. ※ Wall polishing work in accident spot where co-workers worked in was finished at about 15:40. At about 16:10, the victim received a work order from the supervisor and finished pillar surface in loading dock on the 1 st floor using a grinding grinder; and At about 16:50, he moved to accident spot to place dismantled scaffold and steel pipes in order; and He went back into the dry area and he was dismantling scaffold and steel pipes on the lower part; ※ At the time of fatality, protective equipment supplied to the workers was stored in a warehouse in the site and the workers were supposed to take their equipment by themselves. The victim, however, was wearing only a safety cap and not wearing a safety harness. He lost his balance and fell down to ramp floor below 10.5m along with scaffold steel pipes to death. 119 rescue services took him to the hospital. However, he died during medical treatment. Not wearing a safety harness - The employer should had installed a safety net for any opening located above 2m high and had workers wear their safety harnesses. However, the rules were not observed. (When it is not easy to install safety handrail, fence, handle, and etc, a safety net must be installed.) No safety harness attachment - The employer should had installed safety harness attachments for high place workers who work on workplace located above height of 2m. however, it was not observed. Wearing safety harness - The employer should install a safety net for any opening located above 2m high and should have workers wear their safety harnesses. However, they were not wearing safety harnesses. (When it is not easy to install safety handrail, fence, handle, and etc, a safety net must be installed.) Installing safety harness attachment - Employers should install safety harness attachments for high place workers who work on workplace located above 2m high.
