WHICH PLANS ? WHO IS ELIGIBLE? Plans: COVA HealthAware COVA Care Eligible Participants: Employee/Retiree ($17/month) Covered spouse ($17/month) Must be enrolled as of July 1, 2015 Up to $34 per month in premium savings

REQUIREMENTS ONLINE HEALTH ASSESSMENT Completion of an online health assessment no earlier than March 1, 2015 BIOMETRIC SCREENING Obtain a Physician Results Form and submit with measurements taken no earlier than April 1, 2013 Don’t forget to use your annual wellness benefits!

REMEMBER… Employee/Retiree and spouse must register separately to complete the online assessment and obtain a physician results form Requirements can be completed at any time during the year -- the the reward will be effective the first of the month after it’s received (fax or postmark date) If no online access, contact ActiveHealth at Enrollments effective on or after August 1, 2015, will not be eligible until July 1, 2016 (FY 2017)
