We have a website developer – Andy Campbell Currently working on: –Brand identification and logo (4/26) –Site map In order to start building the site itself, we need to organize the contents of the site. We need: –Resources –Patient and provider stories Website Development
Resources
Campaign Resources This campaign is positioned to be a resource and communications hub: What can each of you provide to support those goals? What resources would be useful for starting a care transitions initiative? What resources would be useful for strengthening an existing care transitions program?
Campaign Resources We also need stories about care transitions from: patients caregivers nurses providers and others These are going to need to be solicited, folks!
Pledges for Support
Pledges will be the primary means of tracking participation and involvement across the state Pledging to the campaign is a commitment to reduce readmissions, improve care transitions and publicly support the campaign. –Care facilities will actively reduce readmissions –Support facilities will support care transitions and efforts to reduce readmissions.
Electronic Pledges + Easy to complete + Easy to send to the campaign - We don’t know who is signing - Ease of completion may result in uninformed pledging and may not represent the entire facility.
Mail-in Pledges + Requires deliberate action by the potential partner + More likely to be agreed on by a larger group of people - Requires printing, filling out and physical mailing - Potential participants are busy and may not bother with multiple steps
Outreach
Campaign Outreach Who needs to know about this initiative? How can we extend our reach? Who can we get to and who are we missing? How are we going to get people involved?
Next Steps
We need to get resources and stories from you –Think about what was useful when your group first started on care transitions –What have you developed to support your progress? This campaign is about building community and working together –We need contributions
