‘The Way Forward’: Strategic Clinical Networks (July 12) NHS England is the host for Strategic Clinical Networks from 1 st April 2013 Four initial areas operating throughout the country: Cancer Cardiovascular ( stroke, diabetes,cardiac, renal) Maternity, Newborn Children and YP Mental health, LD, Autism, Dementia and Neurological conditions. Plus Respiratory Conditions ( EoE)
Why Clinical Networks? Networks are positive ways for all partners, along a broad continuum, to be involved in planning & improving care & service delivery Networks have been shown to be an effective mechanism to ensure collaboration, joint decision-making and shared learning Networks are a proven model to promote the use/uptake of clinical experience, knowledge and evidence-based clinical pathways to reduce clinical variation & improve care
SCNs in England 12 SCN areas, broadly based around major patient flows to tertiary providers
Purpose of the SCN is: 11 To identify innovation, encourage early adoption of evidence and best practice that can produce a substantial improvement in health outcomes across all 5 domains
Benefits to Commissioners Offer clinical leadership and expertise Help with systematic, methodological pathway review Interpretation of national guidance based on local needs/plans and priorities Facilitate collective effort & engagement to bring about the changes that are needed Advice and suggest what actions & models of care may be needed to improve outcomes Signpost to people, tools & documents Increased opportunities for education, learning, sharing best practice etc
SCN Work Programme Five Specific Objectives to focus on : 1.Reduction in mortality 2.LTCs 3.Integration of care to support early and timely discharge; Supporting the frail and vulnerable to remain in their communities 4.Pathway Reviews supporting commissioners 5.Parity of Esteem
