Palliative Care/End of Life Identified Area for Improvement: 1) A scoping exercise to map the range of end of life / palliative care services in rural mid Wales including: Specialist palliative care services Community health and social care packages for palliative and end of life care In patient resources for end of life and palliative care i.e. Community Hospitals, District General Hospitals & Hospice inpatient care Third sector services 2)The provision of evidence based examples of best practice service models for end of life /palliative care services (cancer and non-cancer) in rural areas

Palliative Care/End of Life Progress to Date: A funding proposal for resource (£44k) to complete a scoping exercise to map End of Life / Palliative Care services in rural Mid Wales has been submitted to the End of Life Board (non-recurrent funding) and been approved. A rapid review of End of Life / Palliative Care models (cancer and non cancer) in rural areas nationally and internationally will be undertaken by the Marie Curie Research Centre at Cardiff University.

Palliative Care/End of Life The Benefits to Mid Wales: The rapid review and scoping exercise will enable the Innovation Sub Group to identify a programme of work that improves the experience of patients, families and their carers by highlighting issues and potential solutions for end of life / palliative care services in rural mid Wales.
