The Lewin Group designed this assessment to assist organizations in evaluating their readiness to perform key functions of Aging and Disability Resource Centers (ADRCs).

© Ingenix, Inc. 9 ADRC-TAE HomeADRC-TAE Home Survey Home Survey Start Page Edit Background InformationSurvey HomeSurvey Start PageEdit Background Information The assessment is: Easy to use--click on the responses that most accurately describe the present status of your organization or agency Confidential--all information is password protected and available only to the user Informative--assesses organizational readiness in seven functional areas of ADRCs and many of the questions include links to additional information or resources about those items Responsive--results are evaluated and immediate feedback is provided Convenient--the survey takes about 30 minutes to complete and, if necessary, you can save your work and come back later.

© Ingenix, Inc. 10 The On-Line Readiness Assessment Survey – Three Easy Steps  Step I -- Provide basic background information about your organization and create a unique password. This will ensure the confidentiality of your responses and will enable you to save your survey and return to it in the future, if needed. The password you create will also be required to access your ADRC Readiness Assessment Results which will appear after you have submitted the completed survey for analysis.

Organization and Governance 2. Personnel Management and Training 3. Service Delivery and Operations 4. Outreach and Marketing 5. Information Technology and Management Information Systems (IT/MIS) Capacity and Support 6. Partnerships 7. Evaluation and Monitoring

These Results will detail your organization's level of readiness in the main areas covered in the survey and offer links to resources that may help your organization build capacity in these areas.
