2 A joint initiative of VCH, PHSA, and PHC What is Clinical & Systems Transformation (CST)? CST is a project to improve patient care by making sure that our practices are evidence-based and consistent across clinical areas, and we are supported by smarter technology including an electronic health record. This will be a huge change in the way we deliver patient care, so dedicated education and support will be provided.

Too many patients suffer from adverse events Too much variation and too many workarounds in patient care Too much time spent chasing paper charts and completing unnecessary paperwork Too many duplicate tests Too much time between an order being written and the patient receiving therapy Too many old, separate clinical information systems “Accurate information is critical to providing good quality and safe patient care.” Vicky Crompton, CST Executive Director and Transformation Lead, PHSA

Evidence-based clinical practices and order sets as standard Powerful new shared clinical information system Computerized provider order entry Closed loop medication management Electronic clinical documentation Clinical decision support CST spans across several areas of the continuum of care including: acute care inpatient and outpatient units, ambulatory care and residential care across VCH, PHSA and PHC.
