26 SF 3112 B SUPERVISOR’S STATEMENT You should ask your supervisor to complete this form. It should be returned to you to submit with the rest of the application. Your supervisor will answer questions regarding your performance, attendance, conduct and any accommodation or reassignment efforts that have been attempted due to illness or injury. A copy of the last performance appraisal and the employee’s position description also needs to be provided by your supervisor.
