Injury profile of deer- vehicle collisions Theresa D. Oey-Devine, M.D., James M. Haan, M.D., Stephen D. Helmer, Ph.D. Department of Surgery, The University of Kansas School of Medicine – Wichita Department of Trauma Services, Via Christi Regional Medical Center – Saint Francis Campus

Injury Profile of DVCs BackgroundPurposeMethods –Inclusion/exclusion criteria Results –Mechanism –Injury pattern –Surgical interventions –LOS –Disposition Summary

Background Deer - #1 cause of animal-related vehicle collisions Kansas ,417 vehicle collisions involved deer (13.3%) –254 injury –5 death 8 th in US 1 in 211 chance to be involved in deer- vehicle collision (DVC)

Purpose To evaluate injury patterns from motor vehicle-deer collisions that result in trauma activation

Methods Retrospective chart review Trauma Database at Via Christi Regional Medical Center – St. Francis Campus Level 1 trauma center Statistical analysis –Quantitative – Anova –Qualitative – Pearson Chi-Square, Fischer’s exact test

Inclusion/Exclusion January 1, 2003 to November 30, 2008 Included: –All patients involved in DVC that resulted in trauma activation or consult Excluded: –Patients treated and released from Emergency Department without trauma activation –Patients refusing transport to the hospital by EMS

Results ParameterValue Subjects66 Age 39.8 (16-80) Gender (Male:Female) 75.8%:24.2% Injury Severity Score (ISS) 5 (median) GCS (admission) 14.7 EtOH15.2% (6.1% >80)

Vehicle Type Closed vs Open Cab

Collision/Swerve

Trauma Activation

MC/ATV Trauma Activation

Injury Pattern - Head + + p = 0.072n = low number n

Injury Pattern - Integument * * p = 0.001

Injury Pattern - Fracture * * p = <0.001n = low number n

Injury Pattern – Thorax/Abdomen * + ++ * p = p = = collision with protective gear

Surgical Intervention n n n = Low number

Length of Stay (LOS) p = n = 66

ICU LOS p = n = 21

Disposition

MC/ATV Disposition

Summary Retrospective chart review –DVC resulting in trauma activation/consult –MC/ATV vs MV/Semi, collide vs swerve –Protective gear MC/ATV, collide, no protective gear –Largest population –Higher trauma level activation –Worse injury profile –More surgical interventions –Deaths

Conclusion MC/ATV that collided with deer have worse injuries and greater risk of death than those that swerve Protective gear plays a role with regards to injury pattern and severity
