 Guest speaker – Allison Wood – Mental Health.  To continue Unit 4- Awareness of communication in Adult Social Care.  To explore the importance of record keeping in Adult Social Care.

 Allison Wood.  Mental Health.

 Records are kept to make sure people are supported safely and consistently.  This is in the way they said they would like to be supported.

 Helps to improve accountability.  Shows how decisions relating to care were made.  Supports the delivery of services.  Supports effective clinical judgements and decisions.  Supports and promotes communication.  Documentary evidence.  Helps to identify risks.  Legal requirement.

 Data Protection Act  Access to Medical Records Act  Human Rights Act  Freedom of Information Act  Communications Act 2003.

 Make sure your handwriting is readable.  All entries should be signed and dated.  Should be accurate, factual and no abbreviations or jargon.  The language should be understandable.  Records should be readable when scanned or photocopied.
